Appointment Request

The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete the appointment request form below. Our scheduling coordinator will contact you to confirm your appointment.

We offer appointment times during regular office hours: Monday-Thursday 8:00am-5:00pm and Friday 7:00am-4:00pm. We always try to accomodate everyones schedule and request that our patients keep their appointments as a courtesy to all of our patients.

Late arrivals are handled on a case per case basis. Missed appointments will be asses a missed appointment fee if 24 hour prior notice is not given. Patients are then allowed to reschedule at their leisure.
